20:51:01:03. Application
requirements. An applicant for registration by examination shall present
the following to the secretary with the application:
(1) The certificate of
registration fee of $35;
(2) A photo of the
applicant that is at least 2¼ by 3¼ inches in size with the applicant's
signature in ink on the back;
(3) A list of the
applicant's practical experience on a form provided by or approved by the
board;
(4) A transcript showing
graduation from a college of pharmacy approved by the American Council on
Pharmaceutical Education; and
(5) A government-issued
form of photo identification.
