64:02:01:11.  Permanent certificates -- Term -- Renewal. Permanent certificates are valid for a period of five years from the date of issue by the department. Renewal certificates may not be issued until the applicant demonstrates continued self-improvement by successfully completing study courses and annual schools or workshops offered by the department.
