67:16:29:02.07.  Augmentative communication device -- Modification -- Prior authorization -- Required documentation. The individual's speech pathologist must obtain prior authorization from the department before an augmentative communication device or a modification to a previously authorized device is provided. The individual's speech pathologist must submit the following information to the department:

          (1)  A copy of the completed certificate of medical necessity which is contained in Appendix C at the end of this chapter;

          (2)  A copy of the physician's order for the device; and

          (3)  An assessment containing the information specified in § 67:16:29:02.09 which has been completed by a speech pathologist or a speech therapist and which may contain input from other health professionals, as necessary.
