67:16:29:02.08.  Requirements for supervising speech pathologist. If services are provided by a speech therapist, the supervising speech pathologist must meet the following conditions:

          (1)  The speech pathologist must assume professional responsibility for the services provided and assure that the services are medically necessary;

          (2)  The speech pathologist must have face-to-face contact with the recipient to evaluate the recipient's medical condition;

          (3)  The speech pathologist must review and approve the assessment prepared by the speech therapist; and

          (4)  The speech pathologist must be available as needed by the speech therapist.
