67:16:29:02.10.  Augmentative communication device -- Maintenance and repair. If the repair or maintenance of an augmentative communication device is expected to exceed $500, the department must authorize the services before they are provided. If seeking prior authorization, the speech pathologist or speech therapist must submit the following information to the department:

          (1)  A description of the problem and the repairs or maintenance needed;

          (2)  A statement as to the effectiveness or estimated durability of the repair or maintenance; and

          (3)  A written estimate of the cost of the repairs or maintenance, including parts and labor.
