44:05:02:01.  Application for license -- Fee. A person or entity that desires to operate an ambulance service must obtain a ground or air ambulance license. The person or entity must submit a completed application at least 30 days before the expected date of operation when applying for a new license or at least 30 days before the expiration of an existing license. Completed applications must be accompanied by a check or money order for $12 payable to the department.
